Kindly complete each question as thoroughly as you can. We will use this information to make the best match with your student(s). You are more likely to be matched if you provide us with much information about yourself. Please fill out as much information as you can so we can get a better idea of who you are and can match you with the appropriate student.

Please review the Guiding Principles below. You will be asked to agree to them in the questionnaire.

Guiding Principles:
  • The program is designed to provide students with a mentor to guide and help them by discussing topics of interest to both parties.
  • The program is not intended to provide students with employment.
  • Participants should expect meeting to take place outside of business hours, in person, on the phone, or on a quick question/answer basis via email.
  • Participants should respect each other's valuable time, and show up for all confirmed meetings.
  • Discussions between mentors and students should be positive, fruitful, and fun!
